The Patient Liaison acts as the main contact and liaison for family/patient for patients with complex scheduling needs and coordination across transdisciplinary care teams.



1. Acts as the main contact and liaison for family/patient for patients with complex scheduling needs and coordination across transdisciplinary care teams.

2. Assesses patient needs, determines appropriate services, and directs to those who can meet care needs.

3. Coordinates appointment scheduling functions and prepares the family/patient for entrance to the department.

4. Performs point-of-service activities [visit preparation, patient tracking].

5. Facilitates patient information and charting.

6. Mentors and acts as resource for care team in patient scheduling and provider support processes.

7. Other duties as required.

Technical Expertise

1. Knowledge of medical and clinical terminology/information.

2. Excellent communication and organizational skills required. 

3. Independent thinker with the ability to multi-task, anticipate problems and resolve problems encountered.

4. Excellent interviewing skills required.

5. Strong interpersonal and listening skills required. 

6. Serves as a role model within the work environment. 

7. Working knowledge of PC Window applications sufficient to input and retrieve data as measured by experience or testing. 

8. Working knowledge of computerized appointment scheduling system, or other related office functions. 

9. Ability to type 35 WPM.

10. Must be certified within six months of hire date in Non-violence Crisis Intervention by the Crisis Training Institute or an equivalent program that addresses ways to de-escalate potentially violent incidents.

Education and Experience
1. Education: High school diploma or equivalent and a minimum of four (4) years relevant experience required. Bachelor’s degree and minimum of one (1) year of relevant experience preferred.  
2. Years of relevant experience: 1 to 4 years, depending on education.

3. Years of experience supervising: None.
