Hematology/Oncology Case Manager

The Hematology/Oncology Clinical Case Manager is a professional nurse who assumes accountability and responsibility for managing the hospitalization of a caseload of both inpatient, and outpatient hem/onc patients. The Case Manager facilitates coordination of care by all members of the health care team including Physicians, Advanced Practice Providers, Nurses and other team members to facilitate achievement of appropriate outcomes.
Graduate from an accredited school of nursing, licensed and currently registered as a professional nurse in the State of Ohio. Bachelor of Science in Nursing required; Masters Degree in Nursing preferred. Previous case management experience pediatric hematology/oncology preferred. Must possess excellent oral and written skills. Demonstrate leadership and interpersonal relationship skills which enhance communication and promote conflict resolution. Demonstrates ability to communicate effectively with parents and the interdisciplinary team. Current Health Care Provider BLS training is required.
