Social Worker II Endo
Shift Information: Day Job
Provides clinical Social Work services to patients and their families through the Endocrine Division and manages patients admitted for DKA. Services include psychosocial assessments, psychosocial interventions, and hospital and community referrals as needed to benefit children with diabetes, Turners Syndrome and other endocrine conditions. Collaborates with multidisciplinary team members, and provides consultation to team members regarding patients’ psychosocial needs and necessary interventions. Collaborates with disability programs, schools, child protective agencies, and community programs. Supports families in adhering to treatment plans. Provides leadership in the planning and implementation of the week long Diabetes Camp. Provides coverage to other outpatient services as needed.
Ohio Licensed Social Worker with Master’s Degree in Social Work from accredited Social Work Academic Program. Prior social work clinical experience with patients with chronic/complex health conditions required. Previous experience working with children and their families. Must have strong clinical skills and demonstrated experience with challenging situations especially non adherence to treatment. Excellent oral and written communication skills required. Must work collaboratively with patient care team and have capacity to work independently and to be proactive and creative in approaches to problem solving.
