Case manages and coordinates the daily care of long-term (> 60 day stay) home care patients and their families. Initiates and updates plan of care for assigned patients on an ongoing basis. Contributes to a comprehensive program of quality assessment and improvement for clinical services. Collaborates with the Clinical Nurse Manager, Clinical Supervisor, Educator, staff, patients/caregivers, providers, and other patient care team members within the community in order to meet the needs patients/caregivers. Serves as a resource person for staff and patients/families.
Graduate from an accredited School of Nursing. Currently licensed as a Registered Nurse in the state of Ohio. Bachelor’s degree in Nursing (BSN) required or must be enrolled in an approved program with expected graduation within 5 years of hire. Minimum of two (2) years relevant experience with at least one (1) year of home care experience is required. Professional certification, computer competency, and pediatric experience preferred. Strong communication (verbal and written), interpersonal and organizational skills required. CPR certification required. Must possess the ability to deal effectively with stress, multiple tasks, noise and interruptions.
