Full - Time: 40 Hours Per Week
2nd Shift: 1pm - 9:30pm
Monday - Friday 
Hybrid: Onsite and Remote Requirements 
Supports the Psychiatric Intake Response Center and Inpatient Behavioral Health Unit


Summary:

This position will serve as a coordinator of care for patients and families, serving as the liaison for ED, inpatient, and addiction services. This position will provide support to identified patients and families with behavioral health needs to triage patients in real time to the appropriate level of care across the hospital system and assist them in navigating barriers to treatment engagement.

Responsibilities:

  1. In conjunction with ED, inpatient, and/ or addiction services providers, help triage patients to appropriate level of care.
  2. Participate in regularly scheduled case conferences to coordinate treatment between team members including, psychiatry, attendings, social work, mental health therapists and other providers.
  3. Assist with navigation between levels of care within Behavioral Health.
  4. Work with family to arrange for behavioral health follow up appointments and identify barriers to engagement in treatment and engage in problem solving to address barriers.
  5. Conduct basic motivational interviewing interventions to assist in addressing barriers.
  6. Provide education about behavioral health conditions to the patients and families as indicated.
  7. Assess for social determinants of health and support referrals.
  8. Coordinate/collaborate with external community partners as needed.
  9. Help with patient throughput throughout the department.
  10. Maintain up to date community resource lists for families.
  11. Other duties as assigned.

Other information:

Technical Expertise

  1. Experience working with multidisciplinary teams.
  2. Proficiency in Electronic Health Record computerized documentation is preferred.
  3. Experience in healthcare is preferred.
  4. Excellent communication skills, both verbal and written, coupled with strong listening skills.
  5. Strong organizational skills with the ability to multi-task, as well as anticipate and resolve problems.
  6. Proficiency in MS Office (Outlook, Excel, Word, Power Point) or similar software is required.

Education and Experience

  1. Education: High school diploma or equivalent is required. Bachelor’s degree in related field preferred or 2-4 years relevant experience
